Saw palmetto (Serenoa repens) is by a wide margin the most researched ingredient in this formula and in the BPH supplement category generally. Multiple clinical trials and meta-analyses have found that saw palmetto extract can improve lower urinary tract symptoms associated with benign prostatic hyperplasia, working in part by reducing the conversion of testosterone into DHT and by lowering inflammation in prostate tissue.
That said, the picture is genuinely mixed, not uniformly positive. Some studies show no significant effect over placebo, and a widely cited large trial found saw palmetto extract did not meaningfully affect PSA levels compared to placebo even at high doses. A 2020 double-blind, placebo-controlled trial published in BMC Urology found that a phytosterol-enriched saw palmetto oil significantly reduced symptom scores and residual urine volume over 12 weeks. Extract standardization appears to affect how consistent results are from product to product.
Pygeum has one of the stronger evidence bases of any ingredient in this formula. A meta-analysis published in the American Journal of Medicine, covering 18 controlled trials and over 1,500 men, found that Pygeum africanum produced a moderately large improvement in combined urinary symptom and flow measures compared to placebo — nocturia reduced by about 19%, residual urine volume dropped by about 24%, and peak urine flow increased by roughly 23%.
Mechanistically, researchers believe Pygeum works by moderately blocking androgen receptor activity and reducing inflammation through a different pathway than saw palmetto's DHT-focused approach, which is likely why the two are frequently combined. Dropout rates and side effects in these trials were similar to placebo.
Pomegranate's prostate research is scientifically interesting but easy to overstate. The strongest pomegranate-and-prostate data comes from phase II clinical trials in men already treated for localized prostate cancer, monitored for rising PSA levels afterward — not from healthy men managing ordinary age-related urinary symptoms. Research through Johns Hopkins' Sidney Kimmel Cancer Center associated pomegranate extract with an increase in PSA doubling time over an 18-month period. Importantly, at least one more recent randomized trial in advanced prostate cancer patients found daily pomegranate juice had no significant impact on PSA levels — results have not been fully consistent even within that specific research population.
In ViriFlow, pomegranate is more reasonably understood as an antioxidant-rich botanical included for general oxidative-stress support, not as a cancer intervention. If you have any personal or family history of prostate cancer, that's a conversation for your doctor.
These four sea vegetables share a common rationale: they're naturally rich sources of iodine, along with fucoidans and other marine polysaccharides studied in general terms for antioxidant and anti-inflammatory activity. Iodine's best-established role in human health is in thyroid function; broader associations between iodine status and prostate tissue health are a less mature area of research compared to saw palmetto or pygeum specifically.

Shilajit is a mineral-rich substance traditionally used in Ayurvedic practice, generally valued for its fulvic acid content and trace mineral profile. It's more commonly studied in the context of general energy and vitality than prostate health specifically.
Neem leaf extract is traditionally used for its general purifying and antimicrobial properties. Its role in this formula appears supportive rather than central.
